This seems to be a fairly general outline that can be used by many in the end of life field, not just psychologists. From the article:
Many factors can impact the decisions people make at the end of life. Below is a categorized list of issues that have been identified in the literature as potentially influencing what particular decisions are made and the process by which such options are selected. The following should be considered a general outline of what a psychologist may want to explore when faced with a person who is trying to make an end-of-life decision, such as voluntarily stopping eating or drinking, withholding or withdrawing treatment, or receiving assisted suicide.
