Ok, this isn't really a toolkit, its list of instruments,surveys and evaluation tools that Institutions can use to many things, including the quality of the end of life care that they are providing. It introduces an active process to institutions and thereby creates change and better care. It is used to "tune-up" or fix something, so toolkit is a pretty good name. Definitely worth a look if you are administrative staff or have an admin role at an institution. The host website Brown University Center for Gerontology and Health Care Research is also worth a look.
